You need to fill out the following forms and bring them with you to your first session:
1. Either an Adult Intake Form OR a Child Intake Form (if your child is under the age of 18)
2. My Office Policy
Additionally, if you are using health insurance, you need to fill out and bring the following form with you:
So, to recap, you should have 3 forms filled out if you’re using insurance, and 2 forms filled out if you’re not.
Also, please be sure to bring:
4. Your insurance card
5. ID card
6. Payment in the form of CASH or CHECK. Please note: At this time, I do not accept credit cards for payment.
If your insurance requires a co-pay, you can figure out what you will owe by looking on the front of your card for “sp” (I’m considered a specialist). The number after that will be the amount you owe as your copay for each session.
Note: If you don’t fill out the forms or forget them, you’ll need to arrive 15 minutes early in order to fill them out in my office.
